Assumption of Risk

I understand that participation in North Texas Speed Academy ("NTSA") speed, agility and conditioning training involves running, sprinting, jumping, change-of-direction work and other strenuous physical activity, and that such activity carries inherent risks including but not limited to muscle strains, sprains, fractures, heat illness, concussion, permanent disability and, in rare cases, death. I knowingly and voluntarily accept these risks on behalf of my child.

Medical Fitness & Authorization

I certify that my child is physically fit to participate and has no medical condition that would prevent safe participation, other than what I have disclosed on this form. In the event of injury or medical emergency, I authorize NTSA staff to secure medical treatment for my child, and I accept financial responsibility for any resulting medical costs.

Release of Liability

In consideration of my child's participation, I release, waive and discharge North Texas Speed Academy, Jonathan Bannister, its coaches, volunteers and facility hosts (including Murphy Middle School and Plano ISD) from any and all claims, demands, or causes of action arising from participation in the program, except in cases of gross negligence or willful misconduct.

Program Rules & Conduct

I agree that my child will follow all coaching instructions, safety rules and facility rules. NTSA may dismiss any athlete whose conduct endangers themselves or others, without refund.

Weather & Cancellation

Sessions run in typical Texas fall heat. NTSA may cancel or reschedule sessions for lightning, extreme weather or facility closure, and will communicate changes by email or text. Program fees are non-refundable once the session has begun.

Photo & Media Consent

Photos and video taken during sessions may be used by NTSA for coaching feedback and promotional purposes. If you prefer your child not appear in promotional media, email coach@ntxspeed.com and we will honor that request.
